Obstetrics at National Maternity Hospital, Dublin

General
This is a full time clinical and research fellowship post of one year’s duration, which is open to all anaesthesia trainees with appropriate medical registration and an interest in obstetric anaesthesia. Ideally, applicants will have attained FRCA, FCAI or equivalent although applicants who have not done so may still be considered.
The National Maternity Hospital was founded in 1894. It is a busy stand-alone maternity hospital located in central Dublin with an annual delivery rate of approximately 7500. The hospital is a tertiary referral centre for complex obstetric cases and is also the national referral centre for patients with placenta accreta spectrum disorder. In addition, there is a busy gynaecology surgery service comprising both day case and short stay surgeries. The hospital has close links to St. Vincent’s University Hospital and University College Dublin.
Successful applicants will join the established anaesthetic department at the National Maternity Hospital and will gain experience in all aspects of obstetric anaesthesia including clinical, audit, research and education skills.
Clinical
The clinical commitment will comprise of two or three weekday shifts per week (8am to 6pm). Depending on level of experience, it is envisaged that the fellow will work autonomously with in-house consultant cover available. Participation in the senior registrar on-call rota is also required, which is approximately 1 in 5 nights and weekends.
Clinical shifts will include;
· Labour ward - attendance at twice daily multidisciplinary rounds, management of labour analgesia and management of parturients with medical problems complicating labour and delivery.
· Theatre - providing anaesthesia for elective and emergency obstetric care in addition to a busy gynaecology surgery service.
· Weekly high-risk anaesthesia pre-assessment clinic and maternal medicine clinic - fellow will gain experience in devising management plans for parturients with complex medical problems.
· Acute pain service and follow up of women after neuraxial anaesthesia. Opportunities for training in chronic pain management for obstetric patients also exist.
Non clinical; fellows will be allocated at least one day per week to non-clinical activities comprising audit, research, education and development of safety protocols. More time may be allocated depending on the level of activity.
Research
Fellows will be encouraged to undertake a research project in an area of interest to them. They will gain experience in writing of study protocols, ethics committee applications, grant applications, patient recruitment, data collection, data analysis and preparation of manuscripts for publication. The fellow will be encouraged to attend and present at suitable local and international anaesthesia meetings.
Education and Simulation
The fellow will participate actively in the teaching program for other anaesthesia trainees, which includes focused transthoracic echocardiography teaching. The fellow will also gain experience in multidisciplinary teaching including regular in-situ multidisciplinary simulation sessions.
Audit and Safety Practices
The fellow will also have the opportunity to undertake an audit and quality improvement project. In addition the fellow will be supported in the development of local safety protocols and guidelines relevant to the National Maternity Hospital, while they may also participate in regular departmental and hospital safety meetings including the postpartum haemmorage review group.
